About The Position

Mercy Home Health/St. Mary Home Care is seeking a Full-time Occupational Therapist for the far Northeast Philadelphia area. This role involves consulting, evaluating, planning, and administering skilled Occupational Therapy services to homebound patients to restore function, relieve pain, and prevent disability. The therapist will collaborate with an inter-disciplinary team to ensure patient needs are met and quality care is achieved. When acting as the patient care manager, the therapist will oversee and provide primary patient care delivery to a select group of patients, supporting a patient-centered care model and maintaining open communication. The role also involves interacting with internal and external care group members as needed.

Requirements

Graduate of an approved OT educational program
Licensure as an OT in the state of PA
One (1) year experience as a professional therapist with Home care experience a plus
Must have current Driver’s license and reliable transportation
CPR certification required

Bachelors or Masters preferred
Home care experience a plus

Responsibilities

Consults, evaluates, plans and administers skilled Occupational Therapy services prescribed by a physician to homebound patients to restore function, relieve pain and prevent disability following illness, disease, injury, or surgery.
Collaborates with Inter-disciplinary team members to assure patient's needs are met, and quality of care is achieved.
When acting as the patient care manager, will manage, oversee and provide primary patient care delivery to a select group of patients supporting the patient centered care model, assuring quality and maintaining open communication.
Interacts with any and all members of the care group both internal and external to the organization as needed.
