About The Position

COREWELL HEALTH VNA is seeking a licensed Occupational Therapist to join their Home Care Team serving West Michigan, specifically Grandville, Holland, and surrounding areas. This role offers autonomy, allowing clinicians to practice at the top of their license while delivering care in patients’ homes. Home health occupational therapy focuses on helping patients regain independence with daily activities and safely function in their living environment. Therapists maintain manageable caseloads, typically seeing 5–7 patients per day, within thoughtfully designed geographic territories to minimize travel and support work-life balance. As part of Corewell Health West, the VNA team is committed to delivering exceptional, patient-centered care across Ottawa County and surrounding communities.

Requirements

• Bachelor's Degree or equivalent from an accredited occupational therapy program
• LIC-Occupational Therapist - STATE_MI State of Michigan Upon Hire required
• CRT-Occupational Therapist, Registered (OTR) - NB-COT National Board for Certification in Occupational Therapy Upon Hire required
• LIC-Driver's License - STATE_MI State of Michigan Home Care Business Lines Upon Hire required
• CRT-Basic Life Support (BLS) - AHA American Heart Association 90 Days required Or CRT-Basic Life Support (BLS) - ARC American Red Cross 90 Days required

Nice To Haves

• CRT-Hand Therapist, Certified (CHT) - HTCC Hand Therapy Certification Commission Hand Therapy Program Upon Hire preferred

Responsibilities

• Provides therapy to patients based on patient need and medical direction.
• Provides patient/caregiver education and arranges for adaptive equipment as needed.
