About The Position

FeldCare Connects is seeking a self-motivated, Cantonese-speaking Occupational Therapist for Home Health to provide premier excellence of care to adults of all ages. This is an Independent Contractor position serving Diamond Bar and surrounding areas. The role involves assessing and evaluating patient needs related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment. Based on these assessments, the Occupational Therapist will develop treatment plans, implement interventions to improve patient well-being, and monitor progress.

Requirements

• Ability and enthusiasm in working with all ages
• Occupational Therapist license and registration by the state
• Completion of an accredited Occupational Therapist program

Responsibilities

• Provide services defined by state laws governing Occupational Therapy practice, in accordance with the plan of care, and in coordination with other healthcare team members.
• Lead patients in therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report patient response to treatment and changes in condition.
• Instruct patients, families, caregivers, and the healthcare team on how they can participate in assisting the patient with therapy.
