About The Position

FeldCare Connects is seeking a self-motivated Occupational Therapist to provide home health services to adults of all ages in the San Rafael area. The Occupational Therapist assesses and evaluates patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, adaptive equipment, and other occupational therapy needs associated with medical conditions. Based on these assessments, the therapist determines a treatment plan, performs interventions to improve patient wellbeing, and evaluates patient progress.

Requirements

• Ability and enthusiasm to work with people of all ages.
• Occupational Therapist license and registration by the state.
• Completion of an accredited Occupational Therapist program.

Responsibilities

• Provide services in accordance with state laws governing Occupational Therapy practice, the plan of care, and coordination with other health care team members.
• Lead patients in therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report patient responses to treatment and changes in condition.
• Instruct patients, families, caregivers, and health care team members in therapy activities they can use to assist the patient.
