About The Position

FeldCare Connects is seeking a self-motivated Occupational Therapist to provide home health care to adults of all ages in the Thermal area. The role involves assessing and evaluating patients’ functional status, activities of daily living, fine motor coordination, home environment, adaptive equipment needs, and other occupational therapy needs related to their medical conditions. Based on each assessment, the therapist develops a treatment plan, provides interventions to improve the patient’s well-being, and evaluates progress.

The Occupational Therapist provides services in accordance with state laws, the plan of care, and coordination with other members of the health care team. The position is for an independent contractor. FeldCare Connects offers clinicians scheduling flexibility, independence, administrative support, and a group focused on helping people.

Requirements

• Occupational Therapist license and state registration.
• Completion of an accredited Occupational Therapist program.
• Ability and enthusiasm to work with people of all ages.

Responsibilities

• Provide occupational therapy services in accordance with state laws, the plan of care, and coordination with other members of the health care team.
• Guide patients in therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report patients’ responses to treatment and changes in their condition.
• Instruct patients, families, caregivers, and health care team members in therapy activities they can participate in to assist the patient.
• Assess and evaluate functional status, activities of daily living, fine motor coordination, home environment, adaptive equipment needs, and other occupational therapy needs.
• Develop treatment plans, provide interventions, and evaluate patient progress.
