About The Position

FeldCare Connects is seeking a self-motivated Occupational Therapist to provide home health care to adults of all ages in Oakland and surrounding areas. This is an independent contractor position in an app-based referral network.

The Occupational Therapist assesses and evaluates patients’ functional status, activities of daily living, fine motor coordination, home environments, adaptive equipment needs, and other occupational therapy needs related to their medical conditions. Based on the assessment, the therapist develops a treatment plan, provides interventions to improve and enhance patient well-being, and evaluates progress.

The therapist provides care in accordance with state laws, the plan of care, and coordination with other members of the health care team. Network clinicians receive administrative support with assignments, communication, scheduling, care coordination, and quality assurance, along with flexibility to set their schedule and preferred work area.

Requirements

• Ability and enthusiasm to work with people of all ages.
• Occupational Therapist license and registration by the state.
• Completion of an accredited Occupational Therapist program.

Responsibilities

• Provide services within state laws governing occupational therapy, in accordance with the plan of care and in coordination with the health care team.
• Lead patients in therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report patients’ responses to treatment and changes in their conditions.
• Instruct patients, families, caregivers, and health care team members in therapy activities they can use to assist the patient.
• Assess patients’ functional status, activities of daily living, fine motor coordination, home environment, adaptive equipment needs, and other occupational therapy needs.
• Develop treatment plans, provide interventions, and evaluate patient progress.
