About The Position

FeldCare Connects is seeking a self-motivated Occupational Therapist to provide home health care to adults of all ages in the North Port area. The Occupational Therapist assesses and evaluates patients’ functional status, activities of daily living, fine motor coordination, home environment, adaptive equipment needs, and other occupational therapy needs related to their medical conditions. Based on these assessments, the therapist develops a treatment plan, delivers interventions to improve patients’ functioning and well-being, and evaluates their progress.

The role involves providing services in accordance with state laws and the patient’s plan of care, coordinating with the health care team, and supporting patients, families, and caregivers in participating in care. This is an independent contractor opportunity offering schedule flexibility, administrative support, and the ability to work in a preferred area.

Requirements

• Ability and enthusiasm to work with people of all ages.
• Occupational Therapist license and registration by the state.
• Completion of an accredited Occupational Therapist program.

Responsibilities

• Provide occupational therapy services in accordance with state laws, the plan of care, and coordination with other members of the health care team.
• Lead patients in therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report patients’ responses to treatment and changes in their condition.
• Instruct patients, families, caregivers, and health care team members in therapy activities they can use to assist the patient.
• Assess and evaluate patient needs related to functional status, activities of daily living, fine motor coordination, home assessments, adaptive equipment, and other occupational therapy needs.
• Develop and carry out treatment plans, and evaluate patient progress.
