About The Position

FeldCare Connects is seeking a self-motivated Occupational Therapist to provide home health services to adults of all ages in the Hawthorne area. This is an independent contractor position. The Occupational Therapist assesses and evaluates patients’ functional status, activities of daily living, fine motor coordination, home environments, adaptive equipment needs, and other occupational therapy needs related to their medical conditions. Based on these assessments, the therapist determines a treatment plan, provides interventions to improve patient wellbeing, and evaluates progress.

Requirements

• Ability and enthusiasm to work with people of all ages.
• Occupational Therapist license and registration by the state.
• Completion of an accredited Occupational Therapist program.

Responsibilities

• Provide services in accordance with state laws governing Occupational Therapy practice, the plan of care, and coordination with other members of the health care team.
• Guide patients in therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report patients’ responses to treatment and changes in their condition.
• Instruct patients, families, caregivers, and health care team members in therapy activities they can participate in to assist the patient.
