About The Position

FeldCare Connects is seeking a self-motivated Occupational Therapist to provide home health services to adults of all ages in the Delano area. The Occupational Therapist assesses and evaluates patients’ functional status, activities of daily living, fine motor coordination, home environment, adaptive equipment needs, and other occupational therapy needs related to their medical conditions. Based on these assessments, the therapist develops a treatment plan, provides interventions to improve and enhance patient wellbeing, and evaluates patient progress.

The therapist provides care in accordance with state laws governing occupational therapy and the patient’s plan of care, coordinating with other members of the health care team. This is an independent contractor opportunity. FeldCare Connects offers clinicians scheduling flexibility, independence, administrative support, and the opportunity to work with a group focused on helping people.

Requirements

• Ability and enthusiasm to work with people of all ages.
• Occupational Therapist license and registration by the state.
• Completion of an accredited Occupational Therapist program.

Responsibilities

• Assess and evaluate patients’ functional status, activities of daily living, fine motor coordination, home environment, adaptive equipment needs, and other occupational therapy needs.
• Determine a treatment plan based on assessment and evaluation.
• Perform interventions aimed at improving and enhancing patients’ wellbeing.
• Evaluate patients’ progress.
• Provide occupational therapy services in accordance with state laws, the plan of care, and coordination with other health care team members.
• Lead patients in therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report patients’ responses to treatment and changes in their conditions.
• Instruct patients, families, caregivers, and the health care team in therapy activities they can participate in to assist the patient.
