About The Position

FeldCare Connects is seeking a self-motivated Occupational Therapist to provide home health care in the Oxnard area and work with adults of all ages. The Occupational Therapist assesses and evaluates patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, adaptive equipment, and other occupational therapy needs defined by medical conditions. Based on these evaluations, the therapist develops a treatment plan, provides interventions to improve and enhance patient wellbeing, and evaluates patient progress.

Requirements

• Ability and enthusiasm to work with people of all ages.
• Occupational Therapist license and registration by the state.
• Completion of an accredited Occupational Therapist program.

Responsibilities

• Provide services defined by state laws governing Occupational Therapy practice, in accordance with the plan of care and in coordination with other members of the health care team.
• Guide patients in therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report patients’ responses to treatment and changes in their conditions.
• Instruct patients, families, caregivers, and the health care team in therapy activities they can participate in to assist the patient.
• Assess and evaluate patient care needs related to functionality, activities of daily living, fine motor coordination, home assessments, adaptive equipment, and other occupational therapy needs.
• Develop treatment plans, perform interventions, and evaluate patient progress.
