About The Position

FeldCare Connects is seeking a self-motivated Occupational Therapist to provide home health care to adults of all ages in the Hickman area. This is an independent contractor position within an app-based referral network.

The Occupational Therapist assesses and evaluates patients’ functional status, activities of daily living, fine motor coordination, home environment, adaptive equipment needs, and other occupational therapy needs related to their medical conditions. The therapist develops a treatment plan based on these evaluations, provides interventions to improve patients’ well-being, and evaluates their progress.

The therapist provides services in accordance with state laws, the plan of care, and coordination with other health care team members. Network clinicians receive administrative support and resources, and can set their own schedules and choose their preferred work area.

Requirements

• Occupational Therapist license and registration by the state.
• Completion of an accredited Occupational Therapist program.
• Ability and enthusiasm to work with people of all ages.

Responsibilities

• Provide occupational therapy services in accordance with state laws, the plan of care, and coordination with other health care team members.
• Lead patients in therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report patients’ responses to treatment and changes in their conditions.
• Instruct patients, families, caregivers, and health care team members in therapy activities they can participate in to assist the patient.
• Assess and evaluate functional status, activities of daily living, fine motor coordination, home environment, adaptive equipment needs, and other occupational therapy needs.
• Determine treatment plans, provide interventions, and evaluate patient progress.
