About The Position

VHA Home HealthCare (VHA) is seeking an Occupational Therapist Clinical Lead for the Adults program in the Scarborough area. This role involves assessing and providing consultation, recommendations, and interventions to adult clients in their homes, using evidence-based best practices. The Clinical Lead will ensure coordinated care delivery through communication with the client care team, problem-solving with clients and teams to promote independence, and upholding VHA's core beliefs and values. This position also includes providing advice, mentoring, consultation, and expertise related to Occupational Therapy. The Clinical Lead will conduct joint visits with other Occupational Therapists and Service Providers, provide input for performance reviews, and support team members facing clinically challenging situations. Additionally, they will provide clinical orientation for new staff, mentor new graduates, and facilitate team peer support meetings. The role involves identifying, promoting, and participating in best practice initiatives, meeting regularly with the Supervisor of Rehab Best Practice, and acting as a resource for implementing and interpreting professional standards of practice. The Occupational Therapist is accountable for assessing and implementing interventions according to VHA and regulatory college standards, implementing client interventions based on assessment and client input, ensuring services are evidence-based and evaluated, documenting all findings, and supporting safe client care through continuous quality improvement.

Requirements

  • Master’s Degree or equivalent in Occupational Therapy from a recognized school of Occupational Therapy.
  • Member in good standing with the College of Occupational Therapists of Ontario.
  • Compassionate and empathetic.
  • Self-driven with ability to work independently.
  • Skilled in caseload management.
  • Skilled at electronic documentation.
  • Able to provide virtual care.

Responsibilities

  • Assessing and providing consultation, recommendations, and interventions to adult clients in their homes using evidence-based best practices.
  • Ensuring coordinated delivery of care through ongoing communication with the client care team regarding plans and schedules for interventions.
  • Problem-solving in collaboration with the client and care team to create opportunities for more independence.
  • Promoting VHA’s core beliefs and values.
  • Providing advice, mentoring, consultation, and expertise related to the Occupational Therapy profession.
  • Providing joint visits with other Occupational Therapists and Service Providers to give input for performance reviews.
  • Supporting Occupational Therapists and Service Providers who face clinically challenging situations.
  • Providing clinical orientation for new Occupational Therapists or those changing clinical areas.
  • Mentoring groups for new graduates or those new to the community setting.
  • Facilitating team peer support meetings.
  • Identifying, promoting, and participating in best practice initiatives reflective of the requirements of the evolving health care system.
  • Meeting regularly with the Supervisor, Rehab Best Practice.
  • Acting as a resource in the implementation and interpretation of discipline-specific professional standards of practice.
  • Contributing to the development of standards of practice, clinical policies, and procedures relevant to the profession.
  • Assessing and implementing occupational therapy interventions in accordance with professional practice guidelines and standards established by VHA and the perspective regulatory college.
  • Implementing client interventions based on assessment information, client input and in conjunction with the client’s integrated care plan.
  • Ensuring that services provided are best practice, evidence-based and evaluated through the routine use of valid and reliable outcome measured.
  • Ensuring coordinated delivery of care through ongoing communication with all members of the client’s team regarding plans and schedules for interventions.
  • Documenting all assessment findings, treatment plans, progress and discharge summaries of interventions as required by the profession, referral source and the organization.
  • Supporting safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
