Home Health OT & COTA – Flexible Schedule | Laie, Oahu
About The Position
Always Best Care Home Health is a growing Medicare-certified home health agency serving patients throughout O'ahu. We're seeking compassionate Occupational Therapists (OTs) and Certified Occupational Therapy Assistants (COTAs) who are passionate about helping patients regain independence and improve their quality of life through personalized in-home therapy. This role offers flexibility, autonomy, and the opportunity to build meaningful relationships with patients. As an Occupational Therapist or Certified Occupational Therapy Assistant, you'll help patients improve their ability to safely perform everyday activities while promoting independence in the home environment.
Requirements
- Active Hawaii Occupational Therapist license (for OT position)
- Minimum one year of clinical experience (for OT position)
- Active Hawaii Certified Occupational Therapy Assistant license (for COTA position)
- Strong communication and documentation skills
- Comfortable with community outreach and educational presentations
- Valid driver's license and reliable transportation
- Ability to work independently while collaborating with the interdisciplinary team
Nice To Haves
- Home health experience preferred but not required (for OT position)
- Minimum one year of clinical experience preferred (for COTA position)
Responsibilities
- Deliver occupational therapy services in patients' homes
- Complete initial evaluations and follow-up visits
- Perform fall risk assessments and home safety evaluations
- Develop and implement individualized treatment plans (OT)
- Assist with treatment interventions established by the supervising OT (COTA)
- Educate patients and caregivers on adaptive techniques and equipment
- Monitor patient progress and communicate changes with the interdisciplinary team
- Complete accurate and timely clinical documentation
