About The Position
Sobe Innovative Rehab is seeking a licensed, practicing Occupational Therapist Assistant to provide outpatient therapy to older adults. The role focuses on helping patients maintain independence, function, dignity, and quality of life through individualized interventions addressing everyday challenges such as mobility, self-care, safety, strength, coordination, and participation in meaningful activities. The assistant will primarily treat patients in an assisted living facility, with occasional home- or community-based visits as needed. The position offers clinical autonomy, flexible patient scheduling within a 9:00 AM–5:30 PM daytime workday, guaranteed hours, and access to experienced clinical support. Documentation must be completed within 48 hours.
Requirements
- Licensed and practicing Occupational Therapist Assistant.
- At least 3 years of experience.

Responsibilities
- Manage a caseload and provide individualized occupational therapy interventions.
- Primarily treat patients in an assisted living facility, with occasional home- or community-based visits as needed.
- Manage the patient schedule within the 9:00 AM–5:30 PM workday.
- Help patients improve independence with daily activities, mobility, safety, and functional skills.
- Address self-care, transfers, balance, coordination, strength, and adaptive strategies.
- Communicate with patients, families, caregivers, and community clinical staff.
- Exercise clinical autonomy while accessing support from experienced therapists as needed.
- Complete documentation within 48 hours.
- Collaborate with the multidisciplinary team.
