About The Position

FeldCare Connects is seeking a self-motivated Occupational Therapist Assistant to provide home health care to adults of all ages in Santa Clarita and surrounding areas. This is an independent contractor position within an app-based referral network.

The posting describes home health occupational therapy responsibilities including assessing and evaluating patients’ functional status, activities of daily living, fine motor coordination, home needs, adaptive equipment, and other occupational therapy needs related to medical conditions. The clinician determines and implements treatment interventions, monitors progress, and works in coordination with the health care team.

Clinicians in the network can make their own schedules and choose their preferred service area. FeldCare Connects provides administrative support with assigning, communication, scheduling, care coordination, and quality assurance.

Requirements

• Ability and enthusiasm to work with people of all ages.
• Occupational Therapist license and state registration.
• Completion of an accredited Occupational Therapist program.

Responsibilities

• Provide services in accordance with state laws governing occupational therapy, the plan of care, and coordination with other health care team members.
• Lead patients in therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report patients’ responses to treatment and changes in their conditions.
• Instruct patients, families, caregivers, and health care team members in therapy activities they can use to assist patients.
• Assess and evaluate patient care needs relating to functional status, activities of daily living, fine motor coordination, home assessments, adaptive equipment, and other occupational therapy needs.
• Determine treatment plans, perform interventions intended to improve patient well-being, and evaluate patient progress.
