About The Position

FeldCare Connects is seeking a self-motivated Certified Occupational Therapy Assistant to provide home healthcare to patients of all ages in the North Port area. This is an independent contractor position within an app-based therapy network connecting partner agencies with therapists.

The assistant addresses patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment. Following the plan of care based on the OT’s assessment and evaluation, the assistant provides interventions to improve and enhance patient well-being and evaluates patient progress. Services are provided in coordination with the healthcare team and in accordance with state laws governing Certified Occupational Therapy Assistants.

Requirements

• Graduation from an accredited Certified Occupational Therapy Assistant program.
• California Certified Occupational Therapy Assistant license.
• Ability and enthusiasm to work with patients of all ages.

Responsibilities

• Provide services within the scope of state laws governing Certified Occupational Therapy Assistants, following the plan of care and coordinating with other healthcare team members.
• Help patients use therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report patients’ responses to treatment and changes in their conditions.
• Instruct patients, families, caregivers, and healthcare team members in therapy activities they can use to assist the patient.
• Provide interventions addressing functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment, and evaluate patient progress.
