About The Position

FeldCare Connects is seeking a self-motivated Certified Occupational Therapy Assistant to provide home healthcare to patients of all ages in the Chino Hills area. This is an independent contractor position within FeldCare Connects’ app-based therapy network.

The role addresses patients’ functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment needs. Following the assessment and evaluation performed by an occupational therapist, the assistant carries out the treatment plan, provides interventions to improve patient well-being, and evaluates patient progress.

FeldCare Connects connects partner agencies with therapists and provides resources and administrative support, including assignment coordination, communication, scheduling, care coordination, and quality assurance. The company emphasizes quality patient outcomes, ethics, integrity, and helping patients.

Requirements

• Graduate from an accredited Certified Occupational Therapy Assistant program.
• Hold a California Certified Occupational Therapy Assistant license.
• Ability and enthusiasm to work with patients of all ages.

Responsibilities

• Provide services within the scope of state laws governing Certified Occupational Therapy Assistants, following the plan of care and coordinating with other healthcare team members.
• Help patients use therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report patients’ responses to treatment and changes in their conditions.
• Instruct patients, families, caregivers, and healthcare team members in therapy activities they can use to assist the patient.
• Carry out the treatment plan based on the occupational therapist’s assessment and evaluation, provide interventions, and evaluate patient progress.
