About The Position

HarmonyCares is a national value-based provider of in-home primary care for people with complex healthcare needs. Nurse Practitioners provide primary care to high-acuity patients in their homes under an integrated, team-based, physician-driven model. The role emphasizes patient care over visit volume and gives clinicians time to address patients’ needs. The position is Monday through Friday, with no holidays or hospital rounds and minimal call responsibilities. Appointment support and a company car with a driver are available in select markets.

Requirements

• Licensed as a Nurse Practitioner in the state where the HarmonyCares Medical Group office is located.
• Federal DEA license or eligibility for one.
• Active CPR/BLS certification.
• Board certification.
• Valid, unencumbered driver’s license and good driving record.
• Directly related experience in home health, geriatrics, hospice, and primary care is sought.

Nice To Haves

• Geriatric training or experience is preferred.

Responsibilities

• Provide primary care to high-acuity patients in their homes.
• Take the time needed to address patients’ needs, focusing on patient care rather than volume.
