About The Position

ArchWell Health is seeking a nurse practitioner to provide outpatient primary care to senior members at neighborhood clinics. The care model emphasizes quality and continuity rather than service volume, with providers spending more time with patients and delivering comprehensive care that addresses medical and social needs. The role is supported by a collaborative, multidisciplinary team and a fully supportive care team. Centers are newly built, community-based, and equipped with cloud-based electronic health record technology. The position may involve travel among nearby centers within the market.

Requirements

• Active, non-probationary license to practice professional nursing or as a Physician Assistant in the state of employment.
• Experience in primary care, including internal medicine, geriatrics, or family medicine.
• Current BLS/ACLS certification.
• Comfort managing a primarily Medicare-aged, medically underserved patient population with comorbid conditions.
• Strong understanding of examination methodologies and diagnostics.
• Interpersonal skills to be approachable and understandable to patients, families, and center team members.
• Confidence delivering high-quality primary care and addressing patients’ medical and social needs.
• Passion for the work and energy for being part of a fast-growing organization.
• Comfort traveling within the market to multiple centers within short distances of each other.

Responsibilities

• Provide outpatient primary care to senior patients.
• Manage a primarily Medicare-aged, medically underserved patient population with comorbid conditions.
• Deliver high-quality care and address patients’ medical and social needs.
• Be available to patients and communicate effectively with patients, families, and center team members.
• Travel within the market to multiple nearby centers.
