About The Position

The Nurse Practitioner within the Nurse-Led Outreach Team (NLOT) program applies advanced practice nursing expertise to provide comprehensive clinical care, including advanced assessment, diagnosis, and treatment, as well as education, consultation, and research in geriatrics. The role improves access to timely, evidence-based medical care for residents in long-term care homes within the Mackenzie Health Hospital catchment, helps decrease avoidable emergency department transfers and hospital admissions, and advances integrated, resident-centered care. It also supports stronger system partnerships, greater capacity and efficiency, reduced wait times and length of stay, and improved information exchange. The Nurse Practitioner contributes to safe, quality care by understanding and working within the scope of practice and associated accountabilities; consulting with or referring clients to other providers when needs exceed the practitioner’s scope or competence; advocating for clients; and collaborating with the health-care team to promote interprofessional, client-centered care at individual, organizational, and system levels. The role supports an inclusive and safe environment and requires readiness to provide both on-site and virtual consultations. The position may require work at any Mackenzie Health site and within 10 long-term care homes in Western York Region.
