About The Position

HarmonyCares is a national value-based provider of in-home primary care services for people with complex healthcare needs. Nurse Practitioners provide primary care to high-acuity patients in their homes in a patient-centered, integrated, team-based, physician-driven model. The role emphasizes addressing patients’ needs rather than focusing on volume. Services are provided Monday through Friday, with no holidays or hospital rounds and minimal call responsibilities. An assistant supports appointments in select markets, and a company car with a driver is available in select markets.

HarmonyCares is headquartered in Troy, Michigan, and operates home-based primary care practices in 14 states. The organization employs more than 200 primary care providers. Compensation is listed at $125,000 to $160,000 per year; individual compensation packages depend on factors including skills, experience, and qualifications.

Requirements

• Licensed as a Nurse Practitioner in the state where the HarmonyCares Medical Group office is located.
• Federal DEA license or eligibility for one.
• Active CPR/BLS certification.
• Board certification.
• Maintain a valid driver’s license and good driving record.
• Directly related experience in home health, geriatrics, hospice, and primary care is sought.

Responsibilities

• Provide primary care to high-acuity patients in their homes.
