Field Nurse Practitioner - Tulsa County, Oklahoma
About The Position
Advantmed is seeking a licensed, dependable Nurse Practitioner to conduct structured in-home wellness and risk-adjustment assessments for diverse patient populations with varying healthcare needs. This PRN, 1099 independent contractor role offers flexible weekday scheduling, consistent visit availability, and operational support. It is intended for Nurse Practitioners seeking ongoing supplemental income rather than short-term or ad hoc work. The role requires ongoing monthly availability of approximately 30 hours per month, responsiveness during onboarding and scheduling, and reliability in honoring confirmed visits and assignments. Providers work independently in the field and travel daily within a 55-mile radius of the assigned county. Most providers complete four to five visits per day, depending on availability. Training, standardized onboarding, equipment guidance, EMR support, scheduling assistance, and clinical escalation support are provided.
Requirements
- Active, unencumbered Nurse Practitioner license in the applicable state.
- Comfort with EMR systems and independent work.
- Maintain a minimum ongoing commitment of approximately 30 hours per month.
- Travel daily within a 55-mile radius of the assigned county.
Nice To Haves
- Three or more years of patient care experience.
- In-home or risk-adjustment experience.

Responsibilities
- Conduct in-home Annual Wellness Visits and chronic condition assessments.
- Perform focused histories, examinations, vital-sign checks, medication reviews, and screenings.
- Conduct Hemoglobin A1C, Spirometry, KED (Kidney Health Evaluation), DRE (Diabetic Retinal Exam), and FIT screenings.
- Educate members on preventive care and chronic disease management.
- Document visits in the EMR to support value-based care and risk adjustment.
- Maintain ongoing monthly availability and be responsive during onboarding and scheduling.
- Honor confirmed visits and assignments.
- Work independently and professionally in the field.
