About The Position

Advantmed is seeking a licensed, dependable Nurse Practitioner to conduct structured in-home wellness and risk-adjustment assessments for diverse patient populations. This PRN independent-contractor role offers flexible weekday scheduling, consistent visit availability, and operational support. Providers are expected to maintain ongoing monthly availability, respond during onboarding and scheduling, honor confirmed visits, and work independently and professionally. The minimum ongoing commitment is approximately 30 hours per month; the role is not intended for casual or very short-term engagement. Daily travel is required within a 55-mile radius of the assigned county. Training, standardized visit guidance, equipment guidance, EMR support, scheduling assistance, and clinical escalation support are provided.

Requirements

Active, unencumbered Nurse Practitioner license in the applicable state.
Comfort with EMR systems and independent work.
Willingness to obtain additional licensure if needed.

Nice To Haves

Three or more years of patient care experience.
In-home or risk-adjustment experience.

Responsibilities

Conduct in-home Annual Wellness Visits and chronic condition assessments.
Perform focused histories, examinations, vital-sign checks, medication reviews, and screenings.
Conduct screenings including Hemoglobin A1C, spirometry, Kidney Health Evaluation (KED), Diabetic Retinal Exam (DRE), and FIT.
Educate members about preventive care and chronic disease management.
Document visits in the EMR to support value-based care and risk adjustment.
Maintain ongoing monthly availability and respond during onboarding and scheduling.
Honor confirmed visits and assignments.
Travel daily within a 55-mile radius of the assigned county.
