About The Position

WellBe Senior Medical is hiring a Nurse Practitioner (APP) to provide value-based primary care directly in patients’ homes in Douglasville, Georgia. The role focuses on whole-person care, autonomy, and building meaningful connections with patients while addressing social determinants of health. The Nurse Practitioner will manage a dedicated patient panel with support from an interdisciplinary care team, including daily team check-ins, ongoing training, and one-to-one mentorship. The position includes a $10,000 sign-on bonus, a salary range of $104,763 to $166,750, mileage, full benefits, and relocation availability. The role involves local field-based travel to patients’ homes.

Requirements

Master’s degree as a Nurse Practitioner.
Active, unencumbered Nurse Practitioner license in Georgia.
Board certification through ANCC or AANP.
Valid driver’s license and a vehicle for local field-based travel.
Applicants must be legally authorized to work in the United States without current or future employer visa sponsorship.
Successfully complete a pre-employment drug screening and comply with the company’s Drug-Free Workplace Policy.
Successfully complete a background check; continued employment is contingent upon satisfactory monthly FACIS checks.
