About The Position

WellBe Senior Medical is seeking a Nurse Practitioner to deliver value-based primary care directly in patients’ homes in Douglasville, Georgia. The role focuses on whole-person care, patient autonomy, and supporting high-risk senior patients through an interdisciplinary care team. The position offers a $10,000 sign-on bonus, a salary of $104,763–$166,750, mileage, full benefits, and relocation availability. Team support includes daily check-ins, ongoing training, and one-on-one mentorship. The Nurse Practitioner conducts comprehensive primary care visits and physical exams in patients’ homes, develops longitudinal chronic-disease care plans, collaborates with the interdisciplinary team to support safe care transitions, and treats acute needs at home to help prevent emergency-room visits. Employment is contingent on successful pre-employment drug screening and background checks. WellBe also conducts monthly FACIS checks on employees. The employer does not provide employment-based visa sponsorship.

Requirements

Master’s degree as a Nurse Practitioner.
Active, unencumbered Nurse Practitioner license in Georgia.
Board certification through ANCC or AANP.
Valid driver’s license and a vehicle for local field-based travel.
Legally authorized to work in the United States without current or future employer visa sponsorship.
Successfully complete a pre-employment drug screening and background check; continued employment is contingent on satisfactory applicable checks and compliance with the Drug-Free Workplace Policy.
