About The Position

WellBe Senior Medical is hiring a Nurse Practitioner to provide value-based primary care directly in patients’ homes in Kankakee, Illinois. The role focuses on autonomous, outcomes-based care for seniors, addressing the whole person and social determinants of health. The position offers a $20,000 sign-on bonus, a salary of $115,239–$183,425, mileage, and full benefits. Practitioners receive daily team check-ins, ongoing training, and one-on-one mentorship, and work with an interdisciplinary care team to support a dedicated patient panel. Employment is contingent upon successful completion of a pre-employment drug screening and background check. The company conducts monthly FACIS checks on employees, and continued employment is contingent upon satisfactory results. WellBe does not provide employment-based visa sponsorship. Employment is at-will unless otherwise specified by contract.

Requirements

Master’s degree as a Nurse Practitioner.
Active, unencumbered Nurse Practitioner license.
Board certification through ANCC or AANP.
Valid driver’s license and a vehicle for local field-based travel.
Must be legally authorized to work in the United States without employer sponsorship now or in the future.
Successful completion of a pre-employment drug screening and background check.

Responsibilities

Conduct comprehensive primary care visits and physical examinations in patients’ homes.
Develop longitudinal care plans to manage chronic diseases for high-risk patients.
Collaborate with an interdisciplinary team to ensure safe, seamless care transitions.
Treat acute needs in the home to help prevent emergency room visits and hospitalization.
