About The Position
WellBe Senior Medical is seeking a Nurse Practitioner to provide value-based primary care to seniors in their homes in Kankakee, Illinois. The role emphasizes autonomous, whole-person care and collaboration with an interdisciplinary team. The position offers a $20,000 sign-on bonus and a salary of $115,239–$183,425, plus mileage and full benefits. Support includes daily team check-ins, ongoing training, and one-on-one mentorship.
Requirements
- Master’s degree as a Nurse Practitioner.
- Active, unencumbered Nurse Practitioner license.
- Board certification through ANCC or AANP.
- Valid driver’s license and a vehicle for local field-based travel.
Nice To Haves
- Master’s degree focus in Adult, Geriatric, Acute Care, and/or Family practice.
Responsibilities
- Conduct comprehensive primary care visits and physical exams in patients’ homes.
- Develop longitudinal care plans to manage chronic disease in high-risk patients.
- Partner with an interdisciplinary team to support seamless and safe care transitions.
- Treat acute needs in the home to help prevent emergency room visits and hospitalizations.
