About The Position
Stafford Services is seeking a Board-Certified Nurse Practitioner to provide care to residents with complex needs in skilled nursing facilities. The Nurse Practitioner will work autonomously as a core member of the clinical team, delivering routine and acute medical care, managing chronic conditions, and supporting quality improvement. The role involves working with multiple facilities and collaborating with facility nursing leadership and attending physicians. The position offers a flexible schedule with a four-day work week consisting of four 10-hour shifts, with no weekends or on-call duties. New graduates may be considered.
Requirements
- Active, unrestricted Nurse Practitioner license in Ohio.
- Active Nurse Practitioner certification.
Nice To Haves
- Skilled nursing or rehabilitation experience is preferred.
- Three or more years of prior registered nurse experience is preferred, particularly for this specialized setting.
Responsibilities
- Provide routine and acute medical care to skilled nursing facility residents.
- Conduct thorough patient assessments.
- Manage complex chronic conditions.
- Oversee personalized care plans.
- Collaborate with facility nursing leadership and attending physicians to provide integrated care.
- Support quality improvement initiatives, particularly efforts to reduce preventable hospital readmissions.
