•Onsite

About The Position

MPAC Healthcare is seeking a Nurse Practitioner to provide patient-centered care to older adults at a designated senior care facility. This autonomous, full-time role makes the practitioner the primary care provider for an average of 12 to 16 patients per day, while collaborating with attending physicians, the facility Director of Nursing, and other providers to improve patient outcomes and maintain continuity of care. The practitioner also supports and leads the nursing team as patients progress through recovery. The role is typically onsite daily, with a standard Monday–Friday schedule and flexibility within a 40-hour work week. Providers generally set a four-hour weekday window for accepting calls and participate in rotating weekend call approximately once every five to six weeks. Providers do not make overnight calls or go into a facility overnight. The position calls for comfort with a changing senior care environment, sound decisions amid ambiguity, and effective teamwork with facility personnel and MPAC colleagues.
