About The Position

Nest Health is hiring a Nurse Practitioner to serve as the primary care provider for families, delivering whole-family primary, acute, chronic, and pediatric care to adults and children in community facilities, through virtual care, and occasionally in patients’ homes. The NP provides care in a culturally congruent, non-judgmental manner and works closely with an interdisciplinary clinical team that includes physicians, behavioral health counselors, registered dieticians, nurses, psychiatrists, and medical assistants. The role combines longitudinal primary and preventive care, care management, and care coordination. The NP uses Nest’s electronic health record and care management tools, participates in team huddles and case conferences, and helps families navigate health and social care services. The position is hybrid, combining travel to rural Arizona communities for in-person care with virtual service delivery. It is a full-time schedule of four ten-hour workdays per week and includes a rotating call schedule with occasional evening and weekend days each month.
