About The Position

WellBe Senior Medical is seeking a Nurse Practitioner to provide value-based primary care to seniors in their homes in Rochester, NY. The role emphasizes autonomous, outcomes-based care, attention to social determinants of health, and support from an interdisciplinary care team. The position includes daily team check-ins, ongoing training, and one-on-one mentorship, with a dedicated patient panel and support for care transitions. The Nurse Practitioner conducts comprehensive home-based primary care visits and physical exams, develops longitudinal plans for high-risk patients with chronic conditions, and treats acute needs at home to help prevent emergency room visits and hospitalizations. Compensation is $110,000–$175,000, plus mileage and full benefits. Relocation is available. Applicants must be legally authorized to work in the United States without current or future employer visa sponsorship. Employment is contingent on pre-employment drug screening and a background check; monthly FACIS checks apply to employees. The role is at-will unless otherwise specified by contract.

Requirements

Master’s degree as a Nurse Practitioner.
Active, unencumbered Nurse Practitioner license.
Board certification through ANCC or AANP.
Valid driver’s license and a vehicle for local field-based travel.
Legally authorized to work in the United States without employer sponsorship now or in the future.
Successfully complete a pre-employment drug screening and background check; ongoing employment is subject to the stated drug-free workplace policy and monthly FACIS checks.
