About The Position

At Brighton/Suncrest Hospice, the goal is to change expectations for hospice care by providing exceptional care and service to patients. The organization allocates resources to increase staff-to-patient ratios, increase clinical visits, and lower clinician caseloads. Suncrest is a Community Health Accreditation Partner (CHAP)-certified hospice.

The Nurse Practitioner performs physical examinations and preventive health measures within prescribed guidelines and physician instructions. The role records physical findings and develops a plan and prognosis based on the patient's condition. The Nurse Practitioner discusses cases with physicians and other health professionals to prepare comprehensive patient care plans, prescribes or recommends drugs or other forms of treatment, and monitors patient progress, reporting it to the supervising physician. The role also collaborates with hospice employees and volunteers as part of an ID. The ideal candidate is available Monday through Friday during daytime hours.

Requirements

• Completion of an accredited Nurse Practitioner program.
• State licensure in good standing.
• Willingness to drive, with reliable transportation, a valid driver’s license, and auto insurance.
• Ability to assist in lifting and transferring patients as required.
• Close vision, distance vision, and ability to adjust focus.
• Availability Monday through Friday during daytime hours is ideal.
