Nurse Practitioner/Physician Assistant: Post-Acute & SNF (Part-Time)
About The Position
Theoria Medical is seeking a part-time Advanced Practice Provider to serve local skilled nursing facilities. The role provides continuity of care for residents in a close-knit, mission-driven community, with flexible scheduling, professional autonomy, technology support, and collaboration with a clinical team. The provider delivers patient-centered care in skilled nursing facilities, including daily rounds, prescribing medications and treatment plans, and addressing acute or emergent issues. Providers communicate with facility nursing staff as needed and maintain reliable on-site attendance for scheduled shifts. Requirements
- Hold an active U.S. license.
- For Nurse Practitioners, hold board certification from ANCC, AANPCB, PNCB, or AACN.
- For Physician Assistants, hold board certification from NCCPA.
- Be able to perform the essential functions of the position satisfactorily, with or without reasonable accommodation.

Responsibilities
- Provide high-quality, patient-centered care to residents in skilled nursing facilities.

- Communicate with facility nursing staff via phone or secure messaging as needed.
- Maintain reliable on-site attendance for scheduled shifts.
- Ensure compliance with clinical standards, facility policies, and regulatory requirements.
