About The Position

GlyCare is seeking a Nurse Practitioner or Physician Assistant to provide inpatient diabetes and glycemic management at Memorial Hermann Northeast Hospital in Humble, Texas. The role focuses on adult hospitalized patients on Med-Surg and Step-Down units, using evidence-based protocols in a collaborative, consultative practice model. Providers work with hospitalists, nursing teams, pharmacy, dietitians, and endocrinology support teams, and document directly in the hospital EMR.

The position offers approximately 16 shifts per month, predictable block-style scheduling, and daytime hospital-based rounding. Minimal after-hours phone-only call may be required. The role does not involve ICU management, procedures, admissions, discharges, or controlled-substance prescribing. Structured onboarding, specialty training, mentorship, ongoing education, and professional development are provided for new graduates and experienced advanced practice providers. A minimum one-year commitment is required.

Requirements

• Active, unrestricted Texas NP or PA license.
• NP certification as FNP, AGNP, or ACNP, or PA-C certification.
• EMR proficiency.
• Recent inpatient experience; strong new graduates with inpatient clinical training are also encouraged to apply.
• Minimum one-year commitment.

Nice To Haves

• Prior inpatient hospitalist experience, outpatient diabetes management experience, or diabetes-focused clinical rotations may qualify candidates for the sign-on bonus.

Responsibilities

• Provide inpatient diabetes and glycemic management consultations.
• Manage adult hospitalized patients on Med-Surg and Step-Down units.
• Adjust insulin and diabetes therapies using evidence-based protocols.
