About The Position

GlyCare is hiring a Nurse Practitioner or Physician Assistant for inpatient diabetes and glycemic management at Memorial Hermann Southeast Hospital in Houston, Texas. GlyCare partners with hospitals to improve glycemic outcomes, reduce complications, and support frontline care teams through a standardized, evidence-based consultative model. Providers work in a collaborative, structured environment with specialty training, mentorship, and predictable daytime rounding schedules.

The role involves approximately 16 shifts per month, with block-style scheduling and more non-working days than traditional Monday–Friday provider roles. Depending on hiring needs, schedules may be Monday–Wednesday plus every other weekend, or Wednesday–Friday plus every other weekend. Work is daytime hospital-based rounding, with minimal after-hours phone-only call. The position does not involve ICU management, procedures, admissions, discharges, or controlled-substance prescribing.

The annual salary range is $124,800–$135,000, with uncapped monthly bonuses based on RVU production. A $3,000 sign-on bonus is available to qualified candidates. A minimum one-year commitment is required.

Requirements

• Active, unrestricted Texas NP or PA license.
• NP certification as FNP, AGNP, or ACNP, or PA-C certification.
• EMR proficiency.
• Recent inpatient experience.
• Minimum one-year commitment.
• Strong new graduates with inpatient clinical training are encouraged to apply.

Nice To Haves

• Prior inpatient hospitalist experience, outpatient diabetes management experience, or diabetes-focused clinical rotations may qualify candidates for the sign-on bonus.

Responsibilities

• Provide inpatient diabetes and glycemic management consultations.
• Manage adult hospitalized patients on Med-Surg and Step-Down units.
• Adjust insulin and diabetes therapies using evidence-based protocols.
• Collaborate with hospitalists, nursing, pharmacy, and acute care teams.
• Document directly within the hospital EMR.
• Practice within a collaborative inpatient consultative model.
