About The Position

Signify Health, part of CVS Health®, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with people in their homes to help bridge gaps in care and build trusted relationships to make people healthier. The goal is to bring flexible, comprehensive, and personalized health evaluations directly to individuals.

The visits, lasting approximately 45 minutes, allow for one-on-one connection and health question answering. They include a review of medication and medical history, a physical evaluation, and potentially diagnostic tests like diabetic eye exams or spirometry if ordered by the health plan. Clinicians will use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter current treatment plans.

Requirements

• Active, unrestricted license(s) in coverage area(s)
• Board certification (required)
• Able to cover the following primary locations: Worcester, MA

Responsibilities

• Conduct In-Home Health Evaluations.
• Engage with people in their homes to bridge gaps in care.
• Build trusted relationships to improve health outcomes.
• Conduct virtual visits as needed.
• Provide all Signify Health services including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
• Be flexible to travel locally and within licensed states, as needed.
