About The Position

Ennoble Care is seeking an experienced Nurse Practitioner or Physician Assistant to provide primary and palliative care to homebound patients in Burlington County, New Jersey. The position applies medical techniques and principles under physician supervision and serves patients with chronic conditions. Ennoble Care is a mobile primary care, palliative care, and hospice provider whose clinicians visit patients at home and offer programs including remote patient monitoring, behavioral health management, and chronic care management.

Requirements

• Board certified, with valid Nurse Practitioner or Physician Assistant certification in New Jersey.
• Strong clinical skills and the ability to accurately and confidently treat, assess, and diagnose.
• Positive demeanor and strong people skills.
• Ability to problem-solve confidently and work independently.
• 1–2 years of primary care experience with geriatric patients.

Nice To Haves

• Hospice experience is highly preferred but not required.

Responsibilities

• Provide high-quality primary and palliative care to patients in and around Burlington County, New Jersey.
• Assess patients’ health through physical examinations and by obtaining, updating, and reviewing medical histories; develop appropriate care plans.
• Communicate effectively with office staff and clinical operations through internal platforms.
• Attend quarterly medical staff meetings.
• Develop clinical impressions, diagnoses, and treatment plans.
• Build trust and consult with patients, families, and caregivers during visits.
• Order diagnostic testing as medically necessary.
• Prescribe medications and supplies and provide appropriate medically necessary interventions.
