About The Position

Ennoble Care is seeking an experienced Nurse Practitioner or Physician Assistant to provide primary and palliative care to homebound patients in Hudson County, New Jersey. The clinician applies medical techniques and principles under physician supervision and cares for patients with chronic diseases. Ennoble Care provides mobile primary care, palliative care, and hospice services, along with programs such as remote patient monitoring, behavioral health management, and chronic care management.

The role involves assessing patients, developing diagnoses and treatment plans, and providing patient-centered care during home visits. The clinician collaborates with office staff and clinical operations, consults with patients and caregivers, and orders medically necessary testing, medications, supplies, and interventions. The position offers a flexible schedule, autonomy, support from an in-house clinical team, and potential growth into the leadership team.

Requirements

• Board certified with a valid Nurse Practitioner or Physician Assistant certification in New Jersey.
• Strong clinical skills and the ability to accurately and confidently treat, assess, and diagnose patients.
• Positive demeanor and strong interpersonal skills.
• Ability to problem-solve confidently and work independently.
• One to two years of primary care experience with geriatric patients.

Nice To Haves

• Hospice experience is highly preferred but not required.

Responsibilities

• Provide primary and palliative care to patients in and around Hudson County, New Jersey.
• Assess patients through physical examinations and by obtaining, updating, and reviewing medical histories; develop appropriate care plans.
• Communicate effectively with office staff and clinical operations through internal platforms.
• Provide high-quality, patient-centered care to a panel of patients.
• Attend quarterly medical staff meetings.
• Develop clinical impressions, diagnoses, and treatment plans.
• Build trust and consult with patients, families, and caregivers during visits.
• Order diagnostic testing as medically necessary.
• Prescribe medications and supplies and provide appropriate medically necessary interventions.
