About The Position

Ennoble Care is seeking an experienced Nurse Practitioner or Physician Assistant to provide primary and palliative care to homebound patients in the Essex County, New Jersey region. The clinician visits patients at home, including people with chronic conditions and limited mobility, and applies medical techniques and principles under physician supervision. The role involves caring for a diverse patient population with chronic diseases and delivering high-quality, patient-centered care.

Requirements

• Board certified, with valid Nurse Practitioner or Physician Assistant certification in New Jersey.
• Strong clinical skills and the ability to accurately and confidently treat, assess, and diagnose patients.
• Positive demeanor and strong people skills.
• Ability to problem-solve confidently and work independently.
• 1–2 years of primary care experience with geriatric patients.

Nice To Haves

• Hospice experience is highly preferred but not required.

Responsibilities

• Provide high-quality primary and palliative care to patients in and around Essex County, New Jersey.
• Assess patients through physical examinations and by obtaining, updating, and reviewing medical histories; develop appropriate care plans.
• Communicate effectively with office staff and clinical operations using internal platforms.
• Provide patient-centered care to a panel of patients.
• Attend quarterly medical staff meetings.
• Develop clinical impressions, diagnoses, and treatment plans.
• Build trust and consult with patients, families, and caregivers during visits.
• Order diagnostic testing as medically necessary.
• Prescribe medications and supplies and recommend appropriate medically necessary interventions.
