Nurse Practitioner or Physician Assistant (PRN) - In-Home Health Assessments

Matrix Medical Network•Village of Ossining, NY
•Onsite

About The Position

Matrix Medical Network seeks PRN Nurse Practitioners or Physician Assistants to conduct comprehensive in-home health assessments for adults, older adults, and other at-risk individuals. During visits of up to one hour, providers review and observe members’ current health, medical history, medication adherence, social environment, and other risks. The assessments offer insight into members’ overall health that may not be captured during routine office visits and can help improve care quality and close multiple care gaps. Providers deliver services at members’ residences, including private homes and Skilled Nursing Facilities. Visits are scheduled around the provider’s availability, with assessments offered Sunday through Saturday from 8 a.m. to 7 p.m. Compensation is based on completed assessments; the posting indicates that providers averaging 20–100 monthly visits earn $1,800–$11,500 per month, at a per-assessment rate of $90–$115 depending on state of residence. Matrix provides onboarding, a preceptor, individual feedback, and access to continuing education. If business needs exist, the company may support additional state licensure and credentialing. PRN providers may transition to full-time roles and receive a full-time sign-on bonus.
