Nurse Practitioner or Physician Assistant - Community Internal Medicine, Geriatrics and Palliative Care (CIMGP)
About The Position
Community Internal Medicine is seeking a compassionate and skilled Nurse Practitioner (NP) or Physician Assistant (PA) to become an integral member of our multidisciplinary primary care team. In this outpatient role, you will provide comprehensive care for established adult and geriatric patients with acute, chronic, preventive, and medically complex healthcare needs. As an advanced practice provider, you will have the opportunity to build meaningful relationships with patients while working alongside experienced physicians and APP colleagues in a highly collaborative environment. This position utilizes a shared panel model, allowing you to partner closely with a physician or seasoned NP/PA to deliver coordinated, patient-centered care.
Requirements

- Proficiency in a shared panel model

- Experience practicing at the top of your license in a supportive outpatient setting

Responsibilities
- Provide comprehensive assessments, evaluations, and ongoing management of adult and geriatric patients.
- Diagnose and treat acute and chronic medical conditions.
- Order, perform, and interpret diagnostic and laboratory testing.
- Prescribe medications, treatments, and evidence-based therapeutic interventions.
- Develop individualized treatment plans focused on quality outcomes and preventive care.
- Educate and counsel patients and families regarding health conditions, treatment options, wellness, and disease prevention.
- Collaborate with physicians, nurses, care coordinators, and other healthcare professionals to ensure seamless, coordinated care.
