About The Position
The Nurse Navigator provides licensed clinical education, care coordination, and relationship-driven support for residents and families across all levels of care within the CCRC. The role supports residents through health changes and transitions and helps maintain and strengthen occupancy in Assisted Living and Memory Care through proactive relationship building, internal pipeline development, and external outreach.
Requirements
- Must possess a current active license to practice as an LPN in the state.
- Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines governing long-term care.
Nice To Haves
- Previous geriatric nursing or long-term care experience is desired.
