About The Position
The Nurse Care RN Supervisor works on the Clinical team at Lifespace Communities’ Beacon Hill community in Lombard, Illinois. The role coordinates resident assessments and care plans, promotes home health services and wellness programs, monitors changes in residents’ needs, and supports the supervision and development of team members. The position focuses on delivering quality, hospitality-oriented services and meaningful, personalized experiences for residents.
Requirements
- Registered Nurse licensure as required by state regulation.
- One or more years of applicable experience.
- Current CPR certification.
Responsibilities
- Review resident referrals and respond to home care service requests and inquiries.
- Coordinate resident assessments and develop care plans in collaboration with physicians and other health care professionals.
- Help implement and maintain care plans.
- Market home health services to residents.
- Promote home care and wellness programs by conducting resident clinics and sponsoring resident trainings.
- Monitor changes in residents’ conditions and update care plans and service contracts.
- Contact physicians and family members as necessary.
- Ensure resident satisfaction through quality services, hospitality, and related initiatives.
- Participate in hiring, supervising, training, performance management, and corrective action.
- Support team member engagement, relations, recognition, and retention.
