About The Position

Lifespace Communities is seeking a Nurse Care RN Supervisor for Beacon Hill in Lombard, Illinois. The role is part of the Clinical team and supports resident home care and wellness services. The supervisor reviews referrals and service requests, coordinates assessments and care plans with physicians and other health care professionals, monitors resident needs, and helps ensure resident satisfaction through quality and hospitality initiatives. The role also participates in hiring, supervision, training, performance management, and team member engagement and retention.

Requirements

• Registered Nurse licensure as required by state regulation.
• One or more years of applicable experience.
• Current CPR certification.

Responsibilities

• Review resident referrals and respond to home care service requests and inquiries.
• Coordinate resident assessments and develop care plans in collaboration with physicians and other health care professionals.
• Market home health services to residents.
• Promote home care and wellness programs through resident clinics and resident trainings.
• Monitor changes in residents' needs and update care plans and service contracts.
• Contact physicians and family members as necessary.
• Ensure resident satisfaction through quality and hospitality services and initiatives.
• Participate in hiring, supervising, training, performance management, and corrective action.
• Support team member engagement, relations, recognition, and retention.
