NP/PA Sharp Extended Care/Care at Home Full-Time (Float) Opening in San Diego, CA
About The Position
Sharp Rees-Stealy Medical Group is seeking an experienced Family Nurse Practitioner or Physician Assistant to join our Sharp Extended Care Department. This is a 1.0 FTE full-time salaried position with a Monday-Friday schedule and one hour for lunch. The role involves 100% outpatient coverage across Sharp Extended Care and Care at Home services. In Sharp Extended Care, you will manage post-acute patients in skilled nursing facilities, including patient rounding, care coordination, and discharge management. For Care at Home, you will conduct approximately 5 to 6 home visits per day, delivering intermittent skilled medical care to homebound patients in their residences. A key aspect of this role is leading advanced care planning conversations and collaborating closely with patients, families, and interdisciplinary care teams. There are no weekends or inpatient care responsibilities.
Requirements
- Minimum of 2 years of full-time NP/PA experience in an outpatient setting.
- Active, unrestricted California NP or PA license required.
- Current board certification in applicable specialty required.
- Must be comfortable working independently.
- Excellent communication skills with the ability to lead goals-of-care and advanced care planning discussions with patients and families.
Nice To Haves
- Experience in geriatrics, post-acute care, hospice, home-based care, or skilled nursing facility (SNF) settings strongly preferred.
