Frontier Behavioral Health's Elder & Adult Services provides in-home and agency clinical assessments and services to clients (Medicaid/Non-Medicaid) 21 years of age or older. This role utilizes the Gatekeeper Case Finding Model to proactively locate, identify, and serve clients in need of support and assistance in their home. Elder Services serves as the primary access point for services and resources that promote independence and enhanced quality of life for vulnerable older adults who are at risk or isolated. The Mental Health Care Provider offers clinical behavioral health services in key life domains including health, education, employment, social/family, cultural, recreation, spirituality/religion, and housing. These services are delivered with a client-centered, trauma-informed approach aligned with the clinician and recovery models to support client progress toward established goals. Services are provided to individuals of all ages in virtual, office, or community-based settings and include areas such as sleep hygiene, mindfulness, emotional regulation, psychoeducation, referrals, cognitive coping, behavioral activation, communication, administering mental health measures, care coordination, crisis assessment and intervention, identifying strengths, and group facilitation.
