About The Position

The Memory Care Support Partner role is a full-time position at Vitalia Active Adult Community at Rockside in Seven Hills, Ohio. This role focuses on providing empathetic and patient-centered care to residents in memory care. The position involves creating human connections, understanding resident interests and abilities, preparing the environment for daily activities, and partnering with residents throughout their day. Key responsibilities include documenting resident care, assisting with team member orientation, and ensuring a positive resident experience.

Requirements

Must be at least eighteen (18) years of age.
High school diploma, or equivalent.
Ability to read, write, understand, and communicate in English with residents.
Positive and energetic attitude.
Computer skills to print documents and enter resident information.
Must be active as this role requires standing, walking, bending, kneeling, and stooping all day.
Ability to frequently lift and/or move items up to 50 pounds.
Must be able to assist residents with sitting, standing, and walking, as well as assisting persons after a fall.
Must be criminally cleared.

Responsibilities

Be responsible for understanding the importance of empathy and creating human connection with residents during their most vulnerable moments.
Discover residents’ interests, abilities, and what makes each resident who they are.
Collaborate to create appropriate engagement plans and implement strategies with patience to ensure success.
Be responsible for setting the stage for a successful day, from preparing dining rooms and libraries to preparing resident rooms for their nighttime routine.
Partnering with residents to achieve all facets of daily life from the moment they wake to the moment they go to sleep.
Be excellent at documenting resident care and changes in condition.
Assist with orientation of team members assigned to memory care neighborhoods.
