About The Position

The Medication History Specialist (MHS) works as an integral member of the Emergency Department team, obtaining and documenting the best possible medication histories for patients identified for hospital admission. The role supports safe, high-quality care by helping prevent medication duplications, omissions, and errors. Medication information is entered into the electronic medical record and used by physicians, pharmacists, nurses, and other healthcare providers to inform treatment plans, medication orders, and discharge planning.
