About The Position

The Medical Social Worker (LMSW) provides medical social work services, psychosocial assessment, and counseling for patients and their families in close conjunction with the physician and other health care team members. This role involves performing assessments, coordinating discharge planning, providing psychosocial services, and collaborating with healthcare team members to ensure optimal patient care. The position requires maintaining professional credentials and staying current with social work principles and best practices.

Requirements

  • Master's Degree in Social Work from an accredited college or university
  • Qualified by education, training or experience to work with the adolescent, adult and/or geriatric patient population as specialty assignment dictates
  • LMSW - Licensed Medical Social Worker - State Licensure in the State of Tennessee Upon Hire

Responsibilities

  • Performs assessment and implements social work services, maintaining competency and meeting age-specific patient needs.
  • Collects key information from patient, family, community agencies and other professional disciplines for social work assessment and makes referrals as appropriate.
  • Determines priorities for social work intervention, with special consideration for discharge planning and patient self-determination.
  • Provides specialized methods of services based on assessment of needs.
  • Documents assessment, including social work recommendations and professional judgment within twenty-four (24) business hours of referral.
  • Reviews admissions to identify patients from nursing homes and other institutions to initiate discharge planning upon admission.
  • Coordinates with assigned areas to begin discharge planning early in the patient's hospitalization.
  • Communicates with community service providers to identify high risk and problem-prone patients/families requiring social work services.
  • Provides supportive counseling to patients/families to enhance coping mechanisms.
  • Utilizes support groups to provide opportunities for ventilation and to normalize individual reactions to illness.
  • Assesses individual and family needs for more intensive psychotherapy and provides appropriate referrals.
  • Provides crisis intervention services to patients, families, and staff.
  • Provides information to patient and families about types and costs of services available to enable informed decisions.
  • Completes social work assessments and plans, documenting patient and family understanding and input.
  • Communicates discharge plan information to appropriate staff and schedules multi-disciplinary team conferences for complex cases.
  • Initiates referrals to agencies when direct referral will enhance service to the patient/family.
  • Assists patients in obtaining appropriate services such as DME, outpatient therapy, hearing/optical appliances, nursing home, home care, and dialysis.
  • Serves as a resource to the multi-disciplinary team in providing information and education about community resources.
  • Helps coordinate arrangements for patients with special transportation needs transferring from the hospital.
  • Maintains an effective relationship with collateral agencies and includes them in multi-disciplinary team conferences when it improves patient care.
  • Communicates and collaborates with healthcare team members in identifying and planning patient and family needs.
  • Provides documentation and utilizes the computer to enter and retrieve medical/patient information, generate statistics, computations, tables, or charts.
  • Presents Quality Council quarterly reports of findings, analysis, and recommendations.
  • Maintains confidentiality of all information in accordance with hospital, HIPAA, compliance, and other regulatory requirements.
  • Prepares and maintains required reports, records, and files for operational, administrative, and compliance purposes.
  • Attends continuing education, staff development, and mandatory programs.
  • Participates in staff training and performance improvement activities and utilizes the results to initiate changes in practices.
  • Participates in orientation of medical staff, residents, and students to promote understanding of hospital policies and procedures.
