Medical Social Work Care Manager & Patient Advocate (LSW/LCSW) - Full Time

Unicity Care Management•Cranford, NJ
•$70,000 - $90,000•Hybrid

About The Position

Unicity Care Management supports individuals and families navigating significant healthcare decisions, including new diagnoses, complicated hospital discharges, and chronic conditions. The Social Work Care Manager and Patient Advocate serves as the primary point of contact for clients and families, managing a deliberately small private-pay caseload with autonomy to exercise professional judgment and build a schedule around client visits. This is a field-based role with regular travel and a hybrid arrangement; it is not an in-office position. The role includes high-touch support, including accompanying clients to medical appointments and other care-related visits as needed, and responding to urgent client needs that may arise outside standard business hours. The care manager conducts comprehensive in-home assessments, develops and updates individualized care plans, coordinates healthcare providers and community resources, supports transitions of care, provides emotional support and crisis intervention, and advocates for clients at appointments, care conferences, and decision points. The position requires independent caseload management, accurate documentation, and collaboration with physicians, providers, and community partners.
